Please use the form below to anonymously report environmental incidents occurring on the UK campus or other UK facilities to the UK Environmental Quality Management (EQM) Department. Examples of such incidents include observations of spills, leaks, or releases of materials (i.e., liquids, gases or solids including asbestos containing materials) into the environment. However, if the observed environmental incident is an emergency use the more immediate communication alternatives found on the Spill Plans and Response page of EQM’s website.
